AI output verification checklist

AI output should be reviewed according to the harm an error could cause. A casual brainstorm needs light review; public claims, customer communication, code changes, prices, privacy statements, and consequential decisions require stronger evidence and named human approval.

Facts and sources

  • Open every source supporting a material claim.
  • Confirm the page says what the output claims.
  • Check publisher, publication date, geography, sample, and methodology.
  • Prefer primary and official sources for product facts, laws, prices, and policies.
  • Label provider benchmarks and case studies as vendor claims.
  • Remove or qualify claims that cannot be verified.

Numbers and calculations

  • Recalculate totals, percentages, rates, and conversions independently.
  • Check units, currency, tax, billing period, and exchange-rate date.
  • Confirm denominators, sample sizes, and whether percentages can be compared.
  • Inspect spreadsheet formulas and code rather than trusting the summary.

Writing and quotations

  • Verify names, titles, dates, product names, and quotations.
  • Check whether a quotation is exact or paraphrased.
  • Remove invented examples presented as real events.
  • Review tone, originality, cultural context, accessibility, and audience fit.

Code and automated actions

  • Review the full diff and all changed files.
  • Run real tests, linting, type checks, and security checks.
  • Protect secrets and restrict network, shell, deployment, and production access.
  • Confirm rollback before enabling autonomous changes.
  • Do not rely only on an agent’s statement that tests passed.

Images, audio, and video

  • Check consent, likeness, trademarks, music, source assets, and commercial rights.
  • Inspect anatomy, text, identity consistency, synchronization, and misleading context.
  • Apply required synthetic-media disclosure.
  • Require native-speaker review for translation and dubbing.

Privacy and security

  • Remove unnecessary personal, confidential, and regulated data.
  • Verify retention, training use, deletion, regional processing, and connector permissions.
  • Check whether the output exposes secrets or private source material.
  • Use the narrowest permissions and a named reviewer.

Final approval

Record the model or tool, date, relevant settings, sources, reviewer, material limitations, and next review date. Use the AI research tools guide for research reports and the writing prompt guide when preparing structured drafts.

High-risk rule: Medical, legal, financial, safety, privacy, security, and other consequential conclusions require qualified human review and current authoritative sources.

Official source check

Official sources used for product facts: platform.openai.com, developers.google.com. These links support named features, access, plan or policy statements; they are not independent benchmark results.

The links above are included because this page names or compares these products. They support the documented product facts only; the editorial judgment, workflow advice and limitations remain our own and are not presented as independent test results.

Who this guide is for

Best fit: readers who need a repeatable workflow for “AI output verification checklist” and can name the desired output, source boundary, and human reviewer. Not fit: situations where an AI output would be accepted without checking facts, permissions, rights, or downstream consequences. Our editorial assessment is to use the steps as a controlled pilot, not as a promise that one tool will work for every team.

Limits and review checkpoints

Limitations: product interfaces, model behavior, plan access, pricing, retention, and regional availability may change. Keep an original copy of important inputs, record the date and tool version, and verify consequential outputs before sharing or publishing. The ChatGPT tool review provides a related product context for the workflow.

Primary source: Official product source. Re-check the vendor page before relying on time-sensitive details.

November 2026: Keep an evidence trail for important AI outputs

A checklist is more useful when it produces a review record. For each consequential output, save the question asked, source material, tool and model named by the interface, relevant settings, date, reviewer, edits made, and the final decision. Store only what the team needs and follow its retention policy; a review log should not become an uncontrolled copy of confidential source data.

Use an evidence ladder. First, distinguish a directly quoted source from a paraphrase or the model’s own inference. Then open the original source and verify the exact passage, date, and context. Recalculate important numbers independently. For a decision claim, record both supporting and conflicting evidence. If the source is missing, outdated, or does not support the statement, remove the claim or label it as an unverified lead rather than polishing it into certainty.

Match review intensity to possible harm. A brainstorming list may need a quick plausibility check; public product claims, customer communication, code, financial figures, safety advice, or personal-data handling need a named reviewer and stronger evidence. For automated actions, inspect the full proposed change and test the rollback path before granting wider permissions. Do not rely on an assistant’s statement that a test passed—open the test result or rerun it.

After launch, repeat checks when the model, prompt, connected data, or workflow changes. NIST describes risk work as Govern, Map, Measure, and Manage across the AI lifecycle. That framework can help teams assign ownership and monitor known risks, but it does not replace applicable law, a security review, or domain expertise. Use our AI research tools shortlist for source-heavy work and the publishing audit guide for editorial review.

Primary source: NIST’s AI Risk Management Framework Core. This guide offers an editorial verification method, not a claim that a checklist can make every AI output safe or correct. Next review: December 31, 2026.
